KETO IS NOT ENOUGH

…and how to instead cycle keto, carbs & fasting and avoid toxins Dear Friends! We have recently been seeing a big uptick in the popularity of keto on the internet, thanks to the release of the Weimbs Lab study on rats. Many people are trying it and are expecting great results just like the rats did. Let me be clear: I do not recommend just doing an ongoing ketogenic diet eating whatever fits the required fat content. I have been on a cyclical, clean, adequate protein, low toxin version of keto thats more specific – and it’s called „bulletproof“. My results in reduction of kidney volume and dramatic increase of GFR were achieved by doing bulletproof. There are some marked differences between the two and I feel the need to address them, because we dont seem to get similar results when people just do „keto“. It’s just not specific enough. Some key differences are: 1. A bulletproof diet omits natural toxins like phytates, oxalates and lectins which can wreak havoc on kidneys, teeth and other organs 2. A bulletproof diet omits toxic oils like safflower, sunflower, rapeseed, canola, etc. which will increase inflammation and thereby decrease GFR. 3. A bulletproof diet incorporates intermittent fasting on most, but not all days. 4. A bulletproof diet incorporates adequate amounts of animal protein from only grass-fed sources like lamb or beef. Adequate means 0.6-1.2g of protein per pound of lean bodymass per day. 5. There is no „Cheat day“ on bulletproof. You can have a carb refeed every couple days eating green zone carbs (1-2 per week for men and maybe 3 for women) but there is no regular day for consuming junk food, grains, fried stuff or any other red-zone foods. 6. A bulletproof diet cycles carbohydrates on several days per week. (sweet potatoes/white rice or soaked and pressure cooked lentils) (Since so many comments came in about this, I will elaborate: a ketogenic diet is a therapeutic intervention to reduce the growth of cysts or cancers. That is a good thing. Many people do fine on this for years on end. Some people however, develop HPA axis dysfunction because it is overstressed from constant ketosis, as well as thyroid or sexual hormone dysfunction. I wish this weren’t the case, but it is. Since we have PKD for the rest of our lives, it is not a good idea to run the risk of destroying our hormonal systems. Anyone can decide for themselves to go on continuous keto and it will reduce PKD cysts. However, once hormonal problems arise, that is a sign that the body for whatever reason (probably gut flora or genetics) does need some amount of carbohydrate to function. This happened to me. After about 3 years of relatively strict keto with minimal carb refeeds, I still ran my testosterone into the ground. My doc strongly advised me to include another carb day or two. And that brought my hormones back up. So while you might be able to do continuous keto, once your hormone levels drop, that is bad news for all your organs, including your kidneys. The best of both worlds therefore is to include carbs on 1-3 days per week, depending on your needs. If you want to do continuous keto, thats awesome. Do it. And if it ever stops making you feel great, maybe add a carb day or two. You might actually do better that way since you are gaining metabolic flexibility and keeping your body on its toes. Since you are still doing keto 4-6 days a week, you will still have the benefits for PKD. Carbs aren’t the enemy. They are a tool just like anything else is.) SHOPPING TIPS for PKD

Many keep asking for specific products and recommendations,so I will now put together a small shopping guide for you this Christmas time. I use all the articles listed here, or similar- and most of them contribute specifically to PKD, but some just contribute to overall health. Lets briefly go through the overview together: The books, of course, are essential if you don’t just want to know WHAT to get, but also WHY. In my opinion, the bulletproof diet is the most comprehensive form of clean keto availableand it contains many details that are important for kidney health. Especially in the area of toxins and antinutrients you can learn a lot here. But also the special emphasis on organic vegetables and pasture raised/grass-fed meat (no, organic is not enough here) is an important point. The two books „Plant Paradox“ and „Longevity Paradox“ go further into detail on antinutrients and are also quite entertaining.

So lets cover the „basics“ of Bulletproof Coffee. This is mold-free coffee mixed with butter and MCT oil. You really have to make sure that everything is prepared without plastic or aluminum, since a particularly large number of substances are dissolved in the water when acidic and fatty or hot substances like coffee are used. You can blend your coffee with a stainless steel hand blender in a large glass mason jaror in a glass blender with a stainless steel plate as the inner bottom. I use the „Magimix Le Blender“. I linked a similar option below. The Coffee is prepared in a glass french press, also linked below. Ingredients: Real Bulletproof „Original“ coffee, whole beans, freshly ground, MCT oil from a glass bottle and Kerrygold/ANKER/Organic Valley grass-fed butter. It is a good idea to add a spoonful of grass fed collagen every now and then to the coffee and to meals, to strengthen the connective tissue, which is particularly weak in PKD.

However, this is only done on days when there is no intermittent fasting in the morningbecause the collagen may interrupt this (science is not settled yet) I also linked „Lakanto“ below because it is simply the best sugar substitute. It combines Erythritol with monkfruit and tastes like heaven. And its guilt-free. Let’s go over a few basic nutritional supplements: In general, I choose products that are bang-for-buck winners for me considering ingredients and effectiveness. As for the two options on magnesium – malate or magnesium citrate. For later stages more citrate might be warranted, to get rid of microscopic kidney stones, for earlier stages you might like malate, because it charges your krebs cycle (energy production) morewhich leads to more of your own citrate production. (More information about this in the post on citrate)

Vitamin D and K2 are always taken together, and not too little: 1000 IU D3 per 25 lbs body weight per day. Add to that at least 200 mcg of K2 MK7. Fish oil is super important for blood pressure and blood clotting as well as cognitive skills. One gram of EPA + DHA a day in total minimum. Bonus points for more! Some take around 8 grams of total fish oil or even more. A good B complex is mandatory and not everyone can tolerate the standard form, hence the completely coenzymated version linked below.

Turmeric: should not be consumed in the standard form using black pepper as an ingredient, the black pepper affects the intestinal gut lining and lets nasty toxins through with the curcumin it is also always contaminated with mold. Dr.Mercola’s „Autophagy Tea“ gets a special category. This was actually developed as an anti-aging tea, helps with sleepand especially boosts autophagy, i.e. the body’s recycling of damaged cells (e.g. those in cysts). This tea is made in the evening before going to bed:

1 cup of chamomile tea (organic), with
1/2 tl quercetin,
1/2 tl garcinia,
1/2 tl glycine and
1 tl Pau d’arco.
Add some Lakanto to taste if necessary (I find it sweet enough due to the glycine) Some other substances that have a positive effect on PKD because they activate mechanisms of action such as AMPK or block mTOR are:

Ginkgo B.,
Berberine,
Quercetin and
Oxaloacetate.

Ubiquinol is the active form of coenzyme Q10 and is generally recommended for kidney problems in order to stimulate the damaged mitochondria (powerhouses of the cell) again. Activated charcoal is a great thing to clean the stomach and intestines of any toxins (and even endotoxins) in the evening (at least 2 hours after each meal) and thus relieve the kidneys. Take some capsules or a tsp to a tbsp in a glass of water. Of course not if you are taking anything else like autophagy tea. 😉 Zinc and copper should always be taken in combination and are fundamentally important minerals that many are missing these days. Hence I linked a combination supplement below. On to some kitchen utensils that serve me very well: – A Glass water kettle with temperature control – better coffee with 90 degrees – and no plastics! – What water?

I choose a strong but affordable under-counter water filter from Aquasana. Does not waste water, but still removes a large part of the medication residues and pollutants, as well as fluoride from the tap water. A reverse osmosis version is available as well. – Kitchen scale – essential for making Bulletproof coffee. Must-have! – Automated cooking: Coffee maker, steamer, halogen oven, instant pot: more nutrients, less work. All plastic-free on the inside. You should really pay attention to this. – Pink Salt – Absolutely essential! Anyone who still uses iodized salt or table salt: throw it out! – Coconut milk without BPA from a carton. No plasticizers, no milk proteins. This one still has potassium sorbate. If you can find an option with carrageenan or gellan or xanthan gum thats in a carton, let me know. – Ceramic pan – just a great item to have. The first coating that really leeches almost nothing into the food.

Conventional non-stick pans, scratched or not, leech fluoride into the food and the air. Not to speak about aluminum pots and pans! Really not good for a limited detoxification system. Finally, a link to a chemical-free soap. I personally do not use any other soaps or shampoos. I also tossed out toothpaste after using an oral probiotic to kill bad bugs in my mouth. Bad breath be gone. Works like a charm. Those who still use toothpaste should at least switch to one of the organic brands like the one below. Dental floss is a must, a stainless steel tongue scraper and an oral irrigator have been with me for a long time and are worth their weight in gold.

I have never had better teeth than now. Another great tip for healthy teeth is regular consumption of bone broth, raw grassfed milk and raw grassfed cheeses. Not everyone tolerates all of these so your results may vary. You may not know this, but teeth can actually heal. Check out the book by ramiel nagel linked below if dental hygiene is a problem area for you. As a bonus: I let a heat lamp shine on my kidneys every morning for at least 10 minutes by sitting with my bare back toward it.

This strengthens mitochondria and mechanistically speaking it should slow down PKD. After that I take a cold shower for at least 60 seconds – this also strengthens the mitochondria. Yes, you can take a warm shower first and do the cold (all the way) afterwards for 30-120 seconds. Try it. It has so many health benefits. The following links to some pages are my personal links. QUERCETIN SLOWS CYST GROWTH

Here’s a neat article for everyone to read. The gist of it?
Injecting Quercetin (one of my favorite plant compounds, which is completely nontoxic and even recommended for general health) into the backs of PKD mice twice a day made the cysts grow at a much slower rate.

For anyone interested, an oral human dose that led to the optimal result there would be around 75mg/kg over the day, consumed with fatty food. So for me (65 kg) thats 10x500mg capsules of pure quercetin divided over 2-3 meals. A standard human dose might be 1-2 capsules. You decide. Anyone interested? 🙂

PHOSPHOROUS CONTENT OF PROTEINS

For those of us who already have to watch their phosphorous intake, as is the case with impaired kidney function, I thought it was a great idea to come up with this list for you. There are plenty of lists out there about the phosphorous content of foods, but they are very uninteresting, since they dont compare foods based on their nutritional value. Thats why I came up with this one, considering protein in this case.

For example, it is true that a serving of cooked lentils might have a lower phosphorous content than a serving of salmon. However, once you increase the portion size of the lentils to match the protein content of the salmon- the salmon actually is lower in phosphorous per gram of protein that you get from it. One of the reasons I don’t recommend a vegetarian/vegan diet. This is important because especially in later stages of kidney disease we have to make sure we get adequate protein daily. Another way to put it: This graph shows how much phosphorous you would be getting, if you were to eat enough of a food to get 100g of protein from it. For lentils that would equal 1.1kg of lentils, for beef its about a pound.

Calculate your needs: 0.6-1.2 g protein per day per lean pound of body weight. So for a 70kg human with 15% body fat that would be 79-158g of protein per day. Lower end on low-activity days, higher end on workout days and the day after. Use a nutritional calculator like cronometer to find out how much of any particular food you have to eat to reach your desired protein target. Use this graph for an overview of the phosphate content of the options. Meats are all similarly low in phosphorous content and grass-fed beef or lamb would be the healthiest option among these.Cooking can reduce it further.

Here are the cronometer settings you can use:

An easy low phosphorous post-workout or meal replacement option could be an organic flavored grass fed whey protein combined with collagen or eggwhite protein powder (pasteurized) in equal parts. The whey is raw and therefore very bioavailable. The eggwhite is slightly cooked and less bioavailable. Its still a great anabolic protein, much better from an amino acid standpoint than plant proteins- and its low phosphate content is a good reason to incorporate it in later stages. Collagen (much like gelatin) is more of an add-on because its amino acid content does not make it anabolic (anabolic = triggering mtor, muscle building).

While canned beans and lentils might seem like a great choice here, there are some problems with this. First, their amino acid ratios are not optimal for humans, so they have to be combined with other plant proteins for optimum assimilation. They are also high in carbs. So they can only be eaten on carb days. if you want to add them and want convenience, buy EDEN brand, they soak and pressure cook their beans and lentils. other brands have to be pressure cooked in your kitchen! If you are going to buy another brand, choose bpa-free lined cans, and pressure cook when preparing them. the best budget option for beans/lentils is buying organic dried beans/lentils and soaking them for 96 hours before pressure cooking them.

05 – Germ-free means Cyst-free

Hey Folks! I hope you are getting settled in with the bulletproof diet and you have made your appointment for getting your data.

Today I am going to share one of my favorite research papers with you. In this study from 1997, scientists tested an interesting theory for PKD on two groups of PKD-mice. They fed one group of mice the standard chow, and the other group got a sterilized alternative, and were raised in a germ-free cage. And, you guessed it, the second group of mice essentially didn’t develop ANY cysts. 🙂

So: the second group of mice never had to deal with any germs, bacteria, mold-toxins, viruses and fungi, so they did not have cysts, although they had PKD. Interesting stuff, right? This is a citation from the study: „raising genetically cystic mice in a germ-free environment essentially eliminated cystogenesis and increased survivorship to nearly 100%“ And this should be enough for all of us to really clean up our diets.

To not eat old or mushy fruits or vegetables with brown spots on them. To not eat pesticide laden meat from factory farming. And to drink really clean water without fluoride or medication residue. Getting a filter is a really, really good idea. Because all this stuff gets stored in cysts- and will make them grow larger.

DEUTERIUM DEPLETION 101

So, this is new. Deterium is a heavy Isotope of water, also called heavy water. In excess, it destroys our mitochondria, kind of like throwing a brick into a washing machine. (Ever saw that video? :-D) Its just too heavy. Deuterium occurs naturally in all water. The deuterium content of our oceans has increased from ~135 ppm to ~155 ppm in the last 10.000 years. Depleting one’s body of deuterium by eating a high – fat diet like bulletproof and adding to that a drink of deuterium depleted water, has resulted in some very good recovery of cancers. (remember, PKD is VERY similar to cancer in most ways) And since we are not adding some new supplement, but taking something out that shouldn’t even be there, I am very interested. What I personally do now is ordering DDW (Deuterium Depleted Water) at 10-25 ppm online and diluting that with 2 parts of filtered water or spring water. I then drink only this water for about 2 months per year. I might do the test thats offered on  ddcenters.com afterwards. The goal is to get ones own body below 130 ppm.

08 – Lowering Blood Pressure Naturally

Keeping your blood pressure low is essential. this one the docs got right. As you may know, a high percentage of the world population nowadays has high blood pressure, with all sorts of reasons related to lifestyle, not PKD. That said, what are the reasons for most of us, if it is not PKD?

There are lots of reasons, mostly metabolic syndrome caused by a bad diet- so rest assured, fixing your diet and going bulletproof is a huge step in the right direction. When you have your diet in check, you should get your minerals in check next. whats that? potassium, calcium, magnesium are the most important for blood pressure.

The RDA for magnesium is 400mg daily. And that is for healthy people to stay healthy, not for sick people to get better. On top of that, most of us are getting nowhere near that. The result? One of the results can be high blood pressure. Mineral testing in the blood is not reliable, because your body will pull everything out of your bones when it needs more. Needless to say, thats bad news for your bones. So what’s the best supplement regimen? It depends on you. Experiment with the dosage.

Start with 400mg of magnesium malate in the morning and evening.

also make sure you are getting adequate potassium to match. Three times that (2400mg in this example) potassium total from food and supplements (possible potassium-aspartate or potassium citrate) would be sensible. Check how much you are getting from foods in cronometer. Since potassium is added in industrial fertilizer, this is the one mineral we still get plenty of from food.

These values are for capsules. if you opt for the cheaper bulk powders, you have to take into account the percentage of mineral in the powder. Look at the box. For example 5 grams of potassium aspartate contain 860mg of potassium. Take potassium divided over the day so you dont overburden your stomach or kidneys. Powders dissolved in liquid are best. If you take capsules, take in the middle of meals.

So you got your minerals in check. Your blood pressure might already drop several points so keep an eye on the meds, if taking. Next one is Omega 3 fatty acids. You can get a blood test to see what is your omega 6 to omega 3 ratio from full blood, or if you cannot get it, from serum. Ideal would be 4:1 (with 4 parts O6) up to 1:1 But in general, if you don’t eat fatty, wild caught fish 3-4 times a week, you should supplement fish oil. Life extension is a pretty trustworthy brand, but do your own research. One website that lists independent lab tests of different brands is www.labdoor.com.

Dosage varies for everyone of us. Look at the EPA and DHA content of your softgels. 900mg of EPA+DHA/day is recommended for coronary heart disease. I would go much higher even. I personally take 2000-3000mg of combined EPA+DHA per day. Check your bottle.

See how you feel. Experiment. Your blood pressure might come down even lower. and lastly… Remember POST#06 and Niacinamide? Well, it turns out that Niacinamide is not only good for inhibiting SIRT1, but it also may lower your blood pressure. Consider taking it if you are in a later stage of PKD.

If you dont want to take Niacinamide, use regular niacin. For the dosing, Niacin is extremely safe and while at the start you might flush like crazy, this is healthy and good for you. www.doctoryourself.com is a great start.

2019 CHECKUP RESULTS

Just finished my annual Kidney checkup! GFR at 121, CRP at 0.3 and kidney size „probably smaller“ – ultrasound is very hard to pinpoint. Could be that its 5mm shorter than last year, BUT margin of error is around 5mm so… lets just stick with my last MRI results which were 7% reduction in kidney volume compared to 2014.

03 – 12 Mechanisms

Hello Friends! Today I’m going to share some research with you in my new infographic. These are 12 mechanisms of PKD that scientists have already learnt to influence in a positive way. And I am showing you how you can do it at home in these posts. We don’t use medication, we use the power of real food in a new, scientific way.

You can see all the fancy names in the infographic, but you don’t have to know them. You just need to know what to do! Mechanism of the Day: mTOR. We already saw yesterday that mTOR can be inhibited by bulletproof intermittent fasting and protein fasting. But that is not all! Other things you can consider are:

– lab-tested toxin-free Coffee. Currently Made by 2 brands. „Bulletproof Coffee“ and „Flowgrade Coffee“. (if your neph says it is a bad idea, show them the 2 studies linked below.) Here is how to make it, perfect for bulletproof intermittent fasting:

– High intensity interval training (which is the most time-efficient form of sports) at the end of your fasting window right before lunch
High Absorption Curcumin (Not the standard stuff you find at most grocery stores)
– Quercetin 500mg twice daily
– Fucoidin (3-6g daily in the mornings)

If you would combine ALL of these, that would make for a pretty effective inhibition of mTOR. But even doing only some of these might be enough for you to change how your cysts grow.
